Is Cornerstone a UK wide organisation?

We are a Scottish based, health and social care organisation. At Cornerstone, we provide care and support through the following branches:

North Lanarkshire Supported Living

North Lanarkshire Community Support

Glasgow, East Dunbarton and Edinburgh

West Dunbartonshire

Additionally, we have an advice and information service, Cornerstone SDS (Self-Directed Support), based in Aberdeen and Aberdeenshire and ensures that individuals in receipt of social care have the information they need to make informed choices about their support package.

What process do I follow to get a care service set up for me/my relative?

Self-Directed Support (Aberdeen and Aberdeenshire)

Anybody can access information and guidance from Cornerstone SDS. In order to access the options of support available under self-directed support, you will need to have a care assessment and care budget in place from your local authority.

North Lanarkshire

Apart from social work contracts, families can also contact us directly for care and support enquiries.

 

Do you have a self referral form?

A self referral form is used in some of our Branches. Email us at [email protected] and we can forward you a copy.

Self-Directed Support (Aberdeen and Aberdeenshire)

Cornerstone SDS does not have a referral form, anybody in the Grampian area can self-refer to access guidance and information.

North Lanarkshire

This branch has a local referral form that is completed as part of the assessment process.

We have a new training/conference room that will be available to rent once COVID-19 restrictions have ceased.

Self-Directed Support (Aberdeen and Aberdeenshire)

Cornerstone SDS offers social work students the opportunity to fulfil their placement at their service in Aberdeenshire. Students will workalongside Cornerstone SDS Advisors, learning about social care policy and how this works in practice.
