Our Scottish Borders Branch has been delivering care and support to young adults (16yrs plus) and older adults with learning and physical disabilities or a lifelong health condition, for over 10 years.
We currently have seven small, dedicated support teams providing care and support across the Scottish Borders Health and Social Care Partnership area.
Our Connects Hub is based in the Scottish Borders in the town of Galashiels, which offers great links to public transport and easily accessible to the main town. The Hub provides a person-centred approach to enable people we support who have a learning disability, physical disability, complex care needs and autism to reach their full potential, in a safe and friendly environment.
The Hub is fully adapted and offers a range of outcome-based activities and learning opportunities, which are both building, and local community based.
Within the Hub we have a fully accessible sensory room, Changing Space, training kitchen, activity space, common room and sensory garden.

Our dedicated Care at Home and Housing Support teams provides support across the Scottish Borders for adults living in their own homes or within the family home. We provide support to ensure people we support can meet positive outcomes in their life; support to maintain their housing tenancy, social support and with key agreed outcomes.

Our dedicated team of support practitioners provides a range of building and community-based opportunities and outcomes, to develop new life skills and leaning, whilst keeping things fun. We have excellent links with the wider local community within the Scottish Borders and we offer a variety of timetable of events throughout the year.
Our team provide individual support and various group activities to support learning, peer support and independent living skills.
